After reading through all the comments, I want to correct my earlier response. The current NY supplement payments ($300) follow the 2.67% state tax withholding rule that another commenter mentioned. This changed in 2025 from previous years where federal tax was also withheld. Your 1099-G showing $0 federal withholding is correct if you opted out of federal withholding on your regular benefits. The $8 difference ($300 - $292) represents only the state tax portion. You'll still need to pay federal taxes on all benefits received when you file.
Just wanted to follow up - I received my corrected 1099-G today! The NYSDOL agent I spoke with (after using that phone service I mentioned) explained that they had a system-wide issue with reporting state tax withholdings on the supplement. My advice is definitely to call them about this. The corrected form makes a big difference for my tax situation.
Has anyone here ever successfully appealed a denial? I'm wondering if that's what I'll need to do next if I can't get through on the phone. Is there a specific form or process for that in NY?
Yes, you can request a hearing to appeal a denial. You have 30 days from the date on your determination letter to request an appeal. The form is online in your NY.gov account under "Forms and Documents" or you can call and request one. Make sure to have documentation ready that supports your case - pay stubs, employment dates, etc. The hearing is usually done by phone with an administrative law judge.
UPDATE: Finally got through this morning! Called right at 8am and only waited about 25 minutes. The agent said they were using my wrong base period and that I DO qualify! She fixed it right there and said I should see my determination update in 2-3 business days. So relieved! Thanks everyone for your help and suggestions.

I actually missed a week of benefits because I couldn't prove I'd been looking for work one week when I was sick.

She just finished the call! It went WAY better than she expected. Took about 30 minutes total. They mostly just confirmed her information was the same and verified her work search activities (thank you all for the heads up on that - she spent yesterday putting together her records). They told her she'll receive her first payment on the new claim within 7-10 days. Really appreciate everyone's advice here!
My sister had same problem but hers finally came yesterday. She requested on Jan 11th so its taking them like a month+ to send these out. Last year only took like a week! Their system is probably broken again like everything else they do...
UPDATE: Finally received my 1099G yesterday! So it took exactly 4 weeks from request to delivery. I also managed to get through on the phone using the dedicated hotline number that someone mentioned above. The rep said they're extremely backlogged this year because of the system update. Hope yours comes soon!

Just so everyone is clear: When the phone system goes silent after saying "do not hang up," your certification is NOT being processed. This is a system error. You need to either: 1. Use the NY.gov website to certify (preferred method) 2. Try the phone system during off-peak hours 3. Try the workaround mentioned above (waiting 2 minutes then pressing 1) 4. Call the main customer service line to speak with an agent The NYSDOL has confirmed this is a technical issue they're working to resolve. Make sure you get your certification completed by your deadline to avoid payment delays. If you miss your regular certification period due to this issue, you'll need to speak with an agent to file for back weeks.

•I understand your frustration. The fastest way right now is definitely to use the website if possible. If you absolutely must speak to an agent, try calling right at 8:00 AM when they open, or consider using one of the call connection services that others have mentioned in this thread. I hope you get through soon!

Update: Just wanted to let everyone know that I started using the website for certification and it's been working perfectly. Much easier than dealing with the phone system! My payments have been processing normally. Thanks to everyone who helped with suggestions!
