Probably unpopular opinion, but just fill it out again. Yes, the system is broken. Yes, it's annoying. But fighting it will take 10x longer than just resubmitting the form. Pick your battles. I spent weeks arguing with them about a duplicate request last year before finally giving in and just doing it again. Form took 15 minutes, arguing took weeks.
Has anyone tried contacting their state representative about this? I emailed mine last week and surprisingly got a response. They said they'd look into it and actually followed up asking for my info to check my specific case.

u need to hop on this FAST. ny unemployment fraud is outta control rn. sooo many people getting hit with this. def contact the ftc at identitytheft.gov to start a case file - they can help with the steps
Yoooo Im dealing with same thing! Get the Unemployment fraud paperwork in ASAP - print everything, take screenshots of all correspondence. Also - use claimyr.com to get through to an agent on the phone. I waited for hours trying to call myself, paid for their service and got through in like 20 min. Worth every penny to get my case moving.

If it makes you feel any better, I was in your exact situation last year and they DID eventually resolve it and I got all the money that was fraudulently claimed in my name. But it took 8 months and about 50 phone calls. Keep detailed records of every single person you talk to, date, time, what they told you. This documentation will help if you need to escalate.
